Free 15 minute Consultation: We offer a free complimentary 15 minute consultation to answer any questions or concerns.

  • Should your care extend beyond our scope of practice, we will assist you by making appropriate referrals. Your well being is our priority and we are here to support you on your journey.

Insurance: We accept most insurances. A superbill is provided to you to submit to your insurance for reimbursement if your insurance is out of network.

  • If Out of Network, a bill is provided so you may be reimbursed by your insurance.

Financial Hardship: Patients experiencing financial hardship may be eligible for a payment plan or reduced fees based on documented need.

Self-Pay: We accept major credit cards and Health Savings Accounts (HSA) or Zelle.

  • Initial Appointment: $150 - $250 (Depending on complexity)

    • Includes evaluation, diagnosis, plan, education and prescription as needed.

  • Follow up appointments: (Includes prescriptions)

    15 minute $75.00

    30 minute $125.00

    60 minute $250.00
